Region IV is one of the Texas Academic Decathlon's strongest regions in terms of competition. Region IV consists of all schools in the San Antonio Metropolitan area, as well as schools in San Marcos, Seguin, and Austin, with the exception of small schools, which compete in Region XI. The largest school district of the the Region is Northside ISD (all nine comprehensive (non-magnet) high schools in the district compete). Four of the five magnet schools in Northside compete, albeit as part of the comprehensive school campus they share (Business Careers competes with Holmes, Communications Arts competes with Taft, Construction Careers competes with Warren, and Jay Science Academy competes with John Jay. Health Careers, a stand alone magnet school, does not field any AD team).

The Current Champions for Region IV is John Marshall High School (Large) and Little Elm High School (Medium).

Holmes has been the Large School Champion in Region IV since it returned to the Region in the 2003-2004 school year up until the 2008-2009 year, when it lost Region IV for the first time to Earl Warren. (previously, Holmes was in the now defunct Region XIV). Earl Warren claimed the title from the 2012-2013 School year up until the 2014-2015 School year when they lost to John Marshall High School in the 2015-2016 School year. Boerne won in Region IV for the first time in 2007 and won again in 2008, but was not able to win the 2008-2009 title due to the fact that Boerne was closed for remodeling. Sam Champion, the new Boerne high school won, but it is to be noted that all of Boerne's students were students at Champion that year. From the 2006-2007 school year to the 2014-2015 school year, Sam Champion (formerly Boerne High School) had a 9 year winning streak that was broken by Little Elm High School, the 2015-2016 school year.

2006, 2007 and 2009 all saw a record 9 schools advance to State, in which only 80 schools compete.

After the 2003 competition, Texas regions underwent a realignment. Alice High School's win in 2003 broke Alamo Heights 12 year winning streak in the region. Alice is located 133 miles south of San Antonio and 45 miles west of Corpus Christi. Thus, Alice was moved to Region III (Corpus Christi region). Oddly, Flour Bluff (180 miles from San Antonio) was moved from Region III to Region IV. After three years in Region IV another realignment sent Flour Bluff back to Region III.
